The “squeeze” technique involves placing pressure usually to the base of the penis when ejaculation is imminent, this has the effect of delaying ejaculation and may be employed several times during sexual intercourse. The “start stop” technique as the name implies is simply the suspension of intercourse for a period until the urge to ejaculate passes. Again, this may be employed several times during intercourse. While these techniques may address the symptoms of premature ejaculation and may address the underlying cause of PE often these solutions may need to be applied on a permanent basis.
Avoiding certain sexual position which create heightened levels of exciting or sensation can also address some of the symptoms of PE.
Sensitivity reduction can also be achieved using anaesthetic gels and creams applied prior to intercourse, creating a desensitising effect, but requires the use of condoms to prevent the anaesthetic effects being passed to the partner.
There are many online solutions being offered. These vary considerably and therefore may not be suitable in all cases or create side effects including nausea, fatigue, diarrhoea and loss of libido. Oral treatments come in several forms however these can be generally placed in two main categories:
1. Selective serotonin reuptake inhibitors (SSRIs) – Paxil, Zoloft and Prozac;
2. Tricyclic Anti-depressants (TCAs) – Anafranil; and
3. Phosphodiesterase or PDE5 Inhibitors – Viagra, Levitra and Cialis.These medications alleviate symptoms of anxiety by creating a barrier to the take up of the serotonin. This in turn addresses another anxiety/depression related symptom – premature ejaculation.
The mechanism of action of SSRIs is linked to their inhibition of neuronal uptake of serotonin (5-HT) in the central nervous system (CNS).
TCAs act in a similar manner to SSRIs in that they inhibit serotonin uptake and so produce a similar side effect being delay in sexual climax. Clomipramine which based on research has been found to be the most effective TCA may provide a more effective solution than SSRIs in some cases.
A group of drugs known as phosphodiesterase inhibitors (e.g. Viagra) may provide some benefit to men with PE even though it is primarily used to address ED, this may be in part due to the fact that men with ED issues may also exhibit PE symptoms and by addressing these symptoms PE is also addressed. Additionally PDE5 inhibitors may be used in conjunction with SSRI type drugs. However this should only be taken based on review by our Doctors.
